So without mods it's always 100%. Actually it was a bug that was long since fixed, but the rumor was pretty much allowed to circulate until they did finally confirm that bites are now and were always intended to be 100% chance. You can get zombie infected without a wound becoming infected, and you can get a wound infection without getting the zombie infection.

Infection notice is just the regular wound infection you can get if the wound is not treated appropriately.
